Katie has been with SDMD for over 7 years. She will be the first person that you speak to when calling to book a surgical consult and will continue to be by your side throughout your surgical procedure. Katie is the go-to person for any questions you may have leading up to your consult, before surgery and during your recovery.

Nonsurgical Patient Coordinator
Cindy is one of our newest members to the SDMD team. Her positive nature and need to fully educate each patient, makes her a great addition.
During your office visit you will have ample time to discuss one-on-one with either Dr. Dayan or Michelle Whitehall, PA-C, a treatment plan that will be most beneficial. You'll also have the opportunity to meet with Cindy to further discuss and schedule the non-surgical options such as, neurotoxins, fillers and laser that may be recommended for you. Cindy is eager to educate patients on the newest and most in demand treatments available at our office.

Otto J. Placik, MD, FACS
Plastic Surgeon
Dr. Placik is certified by the American Board of Plastic Surgery, and he is an active member of The American Society of Plastic Surgeons. After completing his doctorate at Northwestern University School of Medicine and his fellowship at St. Joseph Hospital in Chicago in the cosmetic reconstruction of complex nasal and facial abnormalities, Dr. Placik continued on to specialize in plastic and reconstructive surgery procedures including breast augmentation, liposuction, and tummy tuck. He also completed an additional fellowship in microvascular and hand surgery at Davies Medical Center. In addition to his hospital appointments at the Northwest Community Hospital in Arlington Heights and Evanston Healthcare—Glenbrook and Evanston Campuses, Dr. Placik works with the Chicago Center for Facial Plastic Surgery.
