Helene Sanders
Office Manager
Helene Sanders is the office manager for SDMD. She attended Howard University in Washington DC with a major in International Business and a concentration in Marketing. Her management experience began with Aveda Salons and Spas; Helene's extensive relationship with Aveda has granted her with profound patience and management skills which are necessary in the beauty industry. The staff has doubled in the past year since Helene has been in this position; she is devoted to helping this still growing company to achieving and surpassing all set goals.
Katie Ziemba
Surgical Patient Coordinator
Katie has been with SDMD for over 7 years. She will be the first person that you speak to when calling to book a surgical consult and will continue to be by your side throughout your surgical procedure. Katie is the go-to person for any questions you may have leading up to your consult, before surgery and during your recovery.

Sarah Ashkan
Nonsurgical Patient Coordinator
Sarah is one of our newest members to the SDMD team. Her caring nature and need to fully educate each patient, makes her a great addition.
During your office visit you will have ample time to discuss one-on-one with either Dr. Dayan or Michelle Whitehall, PA-C, a treatment plan that will be most beneficial. You'll also have the opportunity to meet with Sarah to further discuss and schedule the non-surgical options such as, neurotoxins, fillers and laser that may be recommended for you. Sarah is always eager to educate patients on the newest and most in demand treatments available at our office.
Sarah's Favorite Treatment: It's a tie. Pixel® because my skin looks so refreshed after my treatment. I love only putting on blush in the morning. I'm also obsessed with filler treatments that create beautiful lips.
Kirstin Smith, MS, PA-C
Physician Assistant
Kirstin Smith, MS, PA-C is a NCCPA board-certified physician assistant and a member of the American Academy of Physician Assistants. Kirstin graduated with a Bachelor's degree in Biology from University of Michigan before receiving her Master's degree in Physician Assistant Practice from Rosalind Franklin University of Medicine and Science. Before becoming a PA, Kirstin explored her passion of medicine and esthetics through a myriad of surgical and non-surgical experiences. Her true compassion and dedication for medicine became apparent during this time. This appreciation of aesthetic medicine drove her to become a member of our team, where she is looking forward to providing the upmost quality care to both surgical and non-surgical patients.

Mireya Velasco
Medical Assistant
Mireya assists the Physicians Asisstants and Dr. Dayan in their processes to ensure that the patients at the Chicago Center for Facial Plastic Surgery have a smooth, seamless experience. Her favorite part of her job is seeing patients leave happy and working with a caring, friendly team. She graduated from Robert Morris University with honors. She is also fluent in Spanish.

Otto J. Placik, MD, FACS
Plastic Surgeon
Dr. Placik is certified by the American Board of Plastic Surgery, and he is an active member of The American Society of Plastic Surgeons. After completing his doctorate at Northwestern University School of Medicine and his fellowship at St. Joseph Hospital in Chicago in the cosmetic reconstruction of complex nasal and facial abnormalities, Dr. Placik continued on to specialize in plastic and reconstructive surgery procedures including breast augmentation, liposuction, and tummy tuck. He also completed an additional fellowship in microvascular and hand surgery at Davies Medical Center. In addition to his hospital appointments at the Northwest Community Hospital in Arlington Heights and Evanston Healthcare—Glenbrook and Evanston Campuses, Dr. Placik works with the Chicago Center for Facial Plastic Surgery.
