As we age, our lips tend to drop and thin. The pink portion of the lip and the border of the lip lose their fullness and voluptuous shape. A full, well defined lip is a signal of youthfulness and fertility. It is a calling signal to the male and found to be attractive. A thinning upper lip can give an appearance of anger or old age. If your lips are thinning, then you may be a candidate for augmentation. Materials used to enhance the lips include Restylane®, Juvederm®, and fat transfer. Today's treatments are softer and create a more natural look.
Temporary treatments take minutes to perform and can be done in an office visit. On average, results can be expected to last between six months and a year. Although some people may have effects lasting well beyond a year. The procedure takes minutes to perform. Either topical or nerve block anesthesia can be first used to prevent discomfort from the procedure. With the advent of the blunt tip cannulas, sharp needles are no longer required for injections. Most patients leave the office with little to no bruising or swelling, and they can immediately return to their normal routine. Permanent enhancement uses filler agents, which are of medical grade quality and have a proven safety record. There is very little associated discomfort, and some patients may benefit from cold compresses and prescribed medicines to hasten the healing process. Whichever path you choose, lip augmentation restores the gentle curves and supple appearance characteristic of attractive, youthful lips.
The best candidate for lip augmentation is a physically healthy man or woman who has realistic expectations and is looking to improve the appearance of their lips by restoring the shape and/or adding fullness. Lip injection procedures often require only a topical anesthetic.

When you look in the mirror and evaluate the mouth areas, you're actually looking at several components, including laugh lines and the appearance of the lips.
The folds from the side of your nose to the side of your mouth are called the nasolabial folds or the melolabial folds, although we usually know them simply as "smile lines" or "laugh lines."
These folds have attachments to deeper tissues and as we age the cheeks start to descend, thanks to gravity and the reduced elasticity in our skin and soft tissues. As the cheek falls, it folds over, much like a belly over a belt; if you pull your cheek skin to the side, the fold may go away or you may still see a visible crease.
Unfortunately, we don't currently have ways to prevent the deepening of the smile/laugh lines.
No topical or oral treatments exist to improve the smile/laugh lines. Over-the-counter creams are ineffective and electrical facial toners used to tighten the skin do nothing to prevent the deepening of the folds.
In terms of treatment, consider that many face-lifting surgical procedures don't address this area well, but this is an area very easily improved with filler products and one of the rather easy signs of aging to reverse.

The lower eyelid has three fatty pockets, all about equal in size, that are involved in any puffiness you see. If you look up to the sky these fatty pockets become temporarily accentuated. After reclining for an extended period of time you may see them more prominently because fluid can accumulate in tissues of the lower eyelid.
Puffy eyelids are most often caused by herniation (bulging) of orbital fat (the fat from within the eye socket) that pushes up against the lower eyelid. As we age, the muscle in the lower eyelid weakens, as does the septum (the "net" holding back the fat) and the fat starts to push out against the weaker lower eyelid.
In addition, the fat of the upper cheek atrophies and the muscles in that upper area weaken, which causes the cheek to sag. This leaves a concave area between the lower eyelid and the falling cheek, which looks like a line or hollow, above which is the lower eyelid. The convex curve of the lower eyelid, because of the bulging of the fat, becomes accentuated and that makes the eyes look puffy and tired. Finally, some people are born with excess lower eyelid fat - it's a trait that runs in families.
While puffy eyelids also can be caused by water retention, allergies, or sinus infections, these are temporary situations that go away when we address the cause. More rarely, we identify an underlying kidney or heart disorder that can cause the eyelids to be puffy. This is why it's important to be evaluated by a physician if what you believe is temporary eyelid puffiness doesn't resolve.
No good way exists to prevent puffy eyelids caused by aging and genetics. Losing weight doesn't seem to have any bearing on this fat, but a reduced-salt diet may prevent puffiness related to water retention. Some people find that sleeping with the head elevated on two pillows as opposed to lying flat may help. This will allow the fluid to drain instead of accumulating in the eyelids.
Puffy eyelids caused by infections or allergies need medical treatments such as antibiotics or allergy medications. In the case of water retention, measures that constrict the blood vessels and reduce swelling may help; these include cold compresses and the cold cucumber "eye pads" we sometimes see in movies. I've known people who swear by hemorrhoid creams. While hemorrhoid creams are not a normal indicated topical agent for puffy eyelids, some of these creams may contain an agent called phenylepherine, which is known to constrict blood vessels. These creams may then reduce fluid in the area and in turn reduce the puffy appearance.
I do not recommend taking powerful diuretic medications; they can cause havoc, because they upset the electrolyte balance, i.e., the balance of sodium and potassium in the body, a potentially dangerous situation.
Excess puffiness around the eyes from fatty pockets requires a surgical procedure (blepharoplasty) to resolve. However, fillers such as hyaluronics (like Restylane and Juvederm) can often be used to improve the appearance of puffy lower eyelids, which is often the result of fat bulging from behind the eyes in combination with sagging or falling of the cheek fat, which leaves a depression or hollow between the cheek and the eyelid. (See Fillers, Part IV.) Sometimes, as mentioned above, this condition is made worse by allergies, sinusitis, or salty foods. Caution:Restylane / Juvederm correction in the eye area is technically complex and you will want to find an expert, experienced injector to treat this area.
Dark circles are the common term we use for what appear to be unevenly darkened skin beneath the eyes.
What we commonly call "dark circles," or what look like dark circles, can come from bulging fat pockets in combination with a hollow below the eye, which together create shadows around the eyes. Excess skin pigmentation or thin skin with the blood vessels visible may also make the skin appear darker.
Sun screens are a must in this area especially if the dark circles are caused by excess pigmentation, which worsens with sun exposure. If the circles are secondary to allergies or sinusitis these conditions need to be treated with antihistamines or antibiotics. If the circles are due to shadows then no real preventive measures exist.
We don't have any good oral or topical treatments for dark circles linked with aging or shadows. For the most part, camouflaging makeup products are the only viable option.
Dark circles are a difficult problem to treat. However, if the dark circles appear to be caused by bulging fat pockets, then a nonsurgical option, such as a filler, may be an option. (See Part IV.) Surgery (blephroplasty) to remove the fatty pocket may also be an option.
Dark circles resulting from tiny blood vessels are also difficult to treat and no good option currently exists. I may offer surgery or a laser procedure if I believe that I can remove the vessels. If the dark circles are due to excess pigmentation, I have found that a chemical peel containing TCA best reduces lower eyelid pigmentation. (See Part IV, Chemical Peels, specifically TCA.)
Caution: Although a nonsurgical mesotherapy injection of a chemical into the fat pocket is being touted by some physicians, this method of treatment is very new and its safety has yet to be confirmed. Likewise, although a laser can be used effectively to treat dark pigmentation elsewhere, it is often hard to get the laser into a good position near the eye to treat this area effectively.

Over a certain age, we know what sagging facial skin is because we see it every time we look in the mirror. Do you see a crease that starts from the corner of the mouth and extends down to the jaw line? This line is called an oral commissure groove, drool line, or marionette line. As we age we sometimes look like we are sad when we are not. In some people, these lines can become very deep. The line extends down to the jaw where there often is a depression we call the pre-jowl depression. This depression sits right in front of the jowl (an extension of fat and soft tissue from the cheek that has fallen over the jaw line).
Sagging skin is an inevitable byproduct of aging and gravity.
We can't really prevent the aging process and we can't change our genetic influences, so we're left with a healthy diet and reducing sun exposure. Of course, as you may have noticed, those who are significantly overweight have full, round faces, and they don't have the hanging jowls or loose skin so common to very thin people. This is the case because facial fat fills out the face much like air in a balloon. However, in no way are we suggesting that we all gain weight! The risks of obesity and fat around the midsection and the behind far exceed any benefit that comes from reducing sagging skin. I only mention this because women on starvation or very restrictive diets may begin seeing sagging skin in their cheeks and neck earlier than others. In other words, any extreme doesn't help slow aging or improve overall health.
We don't have effective topical or oral treatments to prevent reduced elasticity in the skin. Eventually skin sagging happens to all of us. However, a good diet rich in antioxidants (see Chapter 2), using sunscreen, and not smoking will help prevent an acceleration of the aging process. Gimmicky facial toners and exercises really are a waste of money.
If the jowl lifts and disappears when the cheek skin is pulled to the side, then this area will respond to facial lifting surgical procedures. (See Appendix A, Plastic Surgery.) Many in their forties, fifties and older begin asking about facial tightening treatments, and surgical facelifting procedures certainly provide the best and most reliable results. The techniques used today are very good and many surgeons perform minimally invasive facelifting procedures that will allow you great results and a quick recovery. Sagging facial skin also can be treated with fillers such as Radiesse, Restylane/Perlane and Juvederm/Juvederm Ultraplus. These filler agents are much like pumping a balloon with air and fill up the deflated face, which provide the appearance of youth. The face also appears lifted.
Today many people avoid or postpone surgical procedures by using fillers often, and the results are very good.
While most nonsurgical facelift procedures using lasers and lightbased devices improve the skin's tone, the procedures rarely significantly lift the skin.
Although some of these nonsurgical procedures have received considerable attention, none have brought predictably good results. In other words, we can't guarantee a good result; while some people get a great result, others see no difference at all. If you decide to undergo one of these no–downtime, nonsurgical facelift procedures, you may be one of the fortunate ones for whom the results are great. But most people find the result subtle and barely noticeable—and disappointing.
Finally, we expect that over the next five to ten years, we'll learn more about the best ways to harness heat and energy emitted from these nonsurgical facelift devices such as RF/Thermage and thread lift.
We are in the relatively early stages of using these devices to lift the skin and in the near future it's likely we'll see a nonsurgical method that predictably and reliably tightens the skin. However, it's unlikely that the results will ever really come close to what we see with a surgical facial lifting procedure.

Perlane is a dermal filler created by the creators of Restylane. This clear gel is created from a hyaluronic acid base. Hyaluronic acid is a natural substance that is already in your body, and it is naturally stimulated during the wound-healing process. The difference between Perlane and Restylane is the size of the particle. Perlane contains larger particle Hyaluronic Acid and ideal for filling deeper lines and wrinkles.
According to clinical studies, the majority of patients who use Perlane maintained their results for at least 6 months.
Perlane is a good choice for those who are seeking to reduce the evidence of volume loss in areas such as the nasal labial folds (the frown lines around the mouth) and the corners of the mouth.

A lot of excitement surrounds the December 2003 it approval of the hylauronic products. Hylauronic acid is a natural substance found in all species that is highly attracted to water with an ability to bind to it. In this way, it fills the wrinkles or lips by attracting water to the site and allowing for a soft, supple correction that looks very natural. Most patients appreciate its long lasting effects, with many experiencing 8 months to a year of correction from a single treatment.
Additionally, it has a very low side effect profile. In our experience, a very small percentage of patients will develop bruising or swelling at the treatment site lasting beyond 24 hours. The majority of people treated are thrilled with the results and appreciate the immediate return to their busy schedule. There have been a few reports of hypersensitivity reactions, but because it is so unlikely, the it does not require skin testing of patients.
The hylauronics are now the filler treatment of choice for correcting the aging changes around the mouth.

Injected Sculptra® can replace deep hollows of the cheeks. In Europe, New-fill is a very popular treatment option to cosmetically fill in deep creases and wrinkles of the face, and U.S. physicians are also using it. Sculptra® is made up of Poly L lactic acid, which is broken down by the body to carbon dioxide and lactic acid. It comes as a powder in a vial that has to be mixed 3 to 36 hours before being injected into the face, below the skin's surface. Immediately following the treatment, most people are elated to see their crease and wrinkles gone. Initial improvement is mostly due to mild swelling, but the product delivers a lasting result.
Over the days following injection, the swelling comes down significantly and most patients will require another treatment in a few weeks. Patients who continue with the product will take 2 to 3 treatments in order to gain significant correction. The results appear to get better over time, and weeks after the treatment, tiny bumps might be felt under their skin, but they shouldn't be visible. Over the last year the recommendations for preparing the product have changed and the occurrence of the bumps has decreased.
The European experience has shown the correction should last approximately 2 years and possibly even longer. There is also evidence that the body will stimulate collagen formation at the treatment site, perhaps providing for some permanent correction. The disadvantages to Sculptra® include the treatment price, and the results are not immediate. If you want to look good by the weekend, we recommend a hylauronic product, but if you have deep hollows around your eyes, cheeks or mouth, Sculptra® may be the best non-surgical alternative.
